As allowed by Workers’ Compensation laws, HACI requires that your first visit to a physician be through one of HACI’s current medical providers.

If you are in Maricopa County, you must go to AIM (Arizona Industrial Medicine) for jobsite injuries whenever safely possible (no risk to life or no risk of worsening injury).

From I-10:
Exit at 16th Street (exit 146). Go south to Villa St, left to Patricio, right to Taylor, left on Taylor. Clinic is straight ahead on 18th Street.
OR:
Exit at Washington St (exit 148). Go west to 18th St, right on 18th St, past Van Buren to clinic on right.
